1. How do I file a wellness claim?
There are no claims forms involved for filing wellness benefits. However, before calling to file a wellness claim, please compile the following information as it pertains to your wellness exam: date of service, procedures and/or tests performed, amount charged, and name and contact information of your medical provider. The procedures covered under your wellness benefit can be found in our claims tab on this site. Click here to go to Claims Information tab. Once you have on hand all of the information to file your claim, please call our Claims Department directly: 1(800) 869-0355, toll free; press 1 and dial ext. 4484.
2. How do I file an accident or sickness claim?
We are sorry to hear about your accident, sickness, or death of a loved one. To file a claim, you will need to complete a claims form. To obtain claims forms, click here, or you can call the claims department directly to have the forms sent to you. Call toll free: 1 (800) 869-0355. Press 1 and dial ext. 4484.
3. I’m not sure which claim form I need.

4. What is my coverage?

8. What are my premium payment options?
Our clients may pay for their benefit programs through payroll deduction, automatic electronic bank draft, or autopay with their credit or debit card. Accounts can be set up through a team member face to face, online, or by telephone. We accept VISA, MasterCard, Discover, and American Express cards. Recurring premium payment options include biweekly, semi-monthly, monthly, quarterly, semi-annual, and annual. If you have questions, please contact us at (844) 872-4968.
9. How do I change my deduction payment method?
To change your deduction payment method, you must complete and sign a new payment authorization form. Click here to obtain the form. Once completed, please send notice via email or fax.
